Thank you for choosing our office for your dental care. We understand that obtaining and maintaining a healthy smile can be costly. We will make every effort to make sure you understand the costs of your treatment and give you tools and information you will need when paying for services.

Our office is happy to file your dental insurance. We will file all dental plans except Medicaid. With your insurance information, we will provide you with an out of pocket estimate due on the day of your treatment. Please keep in mind that even with this estimated service, any treatment not covered by your insurance will be your responsibility. You can be guaranteed that Dr. Burns and staff treat all patients based on dental need and not based on your dental plan coverage.

NO INSURANCE?

We are happy to offer our non-insured patients a way to help reduce the costs of dental care with our in-office membership plan. This plan is not insurance, but simply a membership within our office for being a loyal patient. The plan is on an annual basis and starts around $20 per month/$240 per year and includes 2 routine cleanings, 2 regular exams, routine xrays, 1 fluoride treatment, 1 emergency exam and a 15% discount on most all treatment performed in our office (such as fillings, crowns, etc). PAYMENT FOR SERVICES

Deductibles, co-pays, and fees for treatment will be due the day of service.

OUTSIDE FINANCING: CARECREDIT

(6 months- no interest for treatment costs of $200-$1,500)
(12 months-no interest for treatment costs over $1,500)
